I worked at a local K-Mart store on the weekend during college At one point I was working the Sporting Goods counter during Hunting Season My training on selling licenses was that if someone answered a question on the form in a disqualifying way I was to call management
Management would inform them that they could not sell them a license, I also had to call them to sell a license since I wasn't a State Licensed Agent, and that management then had to report the denial to the police

The shooters recent history in the last few months is littered with so many time there should have been intervention and treatment but everything failed in that

One issue could be that New York State Police never reported, likely not required, that they had an incident with a mentally ill Maine resident that had been committed for evaluation or they deferred to the Army, since he was in State as a reservist, to deal with him
Still a lot of unknowns but what we are finding out is huge cracks that let the shooting happen
